As part of an Essex-wide migration of records, we supported mid-Essex GP practices to move onto new clinical systems, following the withdrawal of old, out of date systems. In addition, there had been major data corruption and failed clinical system backups in the legacy system migration.
We migrated all GP records onto the new system, ensured corrupted files were fixed and carried out extensive system and business change training for end users.
Practices now benefit from having one shared record for each patient across primary care, community care, palliative care, end of life and integration with 111 and out of hours. This has led to more joined-up care between services.
We are now rolling out a similar programme, improved with our learning, across the rest of Essex and Norfolk.
